All claims must be reported with an ICED-9-CM code number and coded to the highest level of specificity for that code. Multiple codes should be entered when necessary to accurately describe the reason for the encounter or service. List first the code for the diagnosis, condition, problem, etc., shown in the medical record to be chiefly responsible for the service provided, then list codes that describe coexisting conditions. Code all documented conditions that coexist at the time of the encounter/visit, and require or affect patient care treatment or management. Do not code conditions that were previously treated and no longer exist. However, history codes (i.e. V10-V19) may be used as secondary codes if the historical condition or family history has an impact on current care or influences treatment. For patients receiving diagnostic services or therapeutic services only, additional diagnoses (e.g. chronic conditions) may be sequenced as secondary to the primary condition, problem or reason for encounter. As always, on professional claims, please be sure to use the appropriate diagnosis code pointer in Block 24e when applicable.
