Early and Periodic, Screening, Diagnosis, and Treatment (EPSDT)

EPSDT checkups are important. They help identify problems in babies, children, and youth up to age 21 before problems become serious. Shots are an important part of the EPSDT check-up to help children stay healthy.

 
A visit to your primary care doctor includes:
  • a health history and physical exam
  • height and weight
  • a development and mental health check
  • tests, such as TB or lead poisoning
  • health education and behaviors to expect during the child's development
  • hearing and vision test
  • nutrition (eating) habit check
  • shots (polio, measles, chickenpox, and hepatitis B) Recommended Immunization Schedule
Other services:
  • dental checkups and treatment
  • treatment and follow-up for problems found during the EPSDT check-up

Check-ups are free. Each time you visit your doctor, nurse or dentist take your VHP CommunityCare (VCC) identification card. Take your child's shot record also.

 
For infants and toddlers, checkups should be given at:
  • At birth
  • 2-4 Days
  • 1 Month
  • 2 Months
  • 4 Months
  • 6 Months
  • 9 Months
  • 12 Months
  • 15 Months
  • 18 Months
  • 24 Months
For older children and youth, checkups should be given at:
  • 3 Years

  • 16 Years
  • 17 Years

For medical appointments, please call the doctor listed on your card.

When calling for an appointment, say the child is a VHP member; give the child's name and VHP ID number. Be sure to take the child's VHP ID card to the office with you.

 
For dental appointments:

Children should start visits to the dentist at age 3 or earlier if there are any dental problems. Dental check-ups are recommended every 6 months. Children do not have to see their PCP before seeing their VHP dentist. VHP Dentists are listed in the VHP Provider Directory, Or you can call Member Services at (615) 782-7878.

 
For vision (eyeglasses) appointments:

Children's eyes also need special care. VHP members under 21 years old can have their eyes checked and get eyeglasses as medically necessary. In addition to annual checkups and eyeglasses, children are entitled to all medically necessary treatments for their eyes that they need.

Children do not have to see their PCP before seeing their VHP eye doctor. VHP eye doctors are listed in the VHP Provider Directory. Or you can call Member Services at (615) 782-7878.
