The
provider to call to setup transportation is changing to: Medical
Transport Services EFFECTIVE 6/01/2003. If you or your
family members do not have a vehicle to transport you to your
medical appointments, you may schedule transportation by calling
1-877-883-2889 prior to your appointment. For transportation
to your Mental Health provider you will need to call Mid-Cumberland
at (615) 333-5445. Usted puede planificar el transporte llamando 1-877-883-2889 y cinco días hábiles antes de su cita. Para el transporte a su proveedor Mental de la Salud que usted necesitara llamar de Cumberland Medio en (615) 333-5445. |
