What does this Notice mean?
VHP CommunityCare cares about the privacy of all of our members. We understand how important it is to keep our members' personal information private and safe. This Notice tells you how VHP CommunityCare protects the privacy of all current and past members.
What kind of member information does VHP CommunityCare collect?
We collect the kinds of personal information that help us take care of our members' health care. We only collect information that helps us better help our members. Sometimes the government asks us to share some things with them. By personal information we mean your Social Security number, date of birth, address and phone number. If anyone in your family is a member of VHP, we will need some of their information too.
How do we protect your privacy?
We allow only trained employees to handle member information. These employees include our claims and medical staff. Employees who do not obey our Privacy Policy will be punished. We are also careful about website security. We have rules that protect any information you give us online.
When would we share Personal Information?
We may share our members' personal information with your ok. Sometimes laws call for us to share certain information. We may share information with health care givers that help VHP help our members. We may share this information with our lawyers, accountants, claims staff and other insurers. Personal information may also need to be shared with law enforcement groups for investigations. Marketing companies that help us tell our members about important things may need to use some of this information. If we share information, those we share it with must protect the information and use it only for the reason given.
How to Correct Your Information:
VHP CommunityCare gives its members the chance to correct the personal information we have collected. If you believe that any personal information we have about you is not correct, please call our Member Services Department at the number on the back of your VHP ID card.
Web Site Privacy Policy:
When you visit our website you will find information that will help you learn about VHP CommunityCare. As a guest of our website we might collect information that tells us how many users are visiting us and where on our website they are going. We use this information to help us make our website better for our guests. We also like to hear your comments and questions. You can e-mail us by clicking on the e-mail link on our website. We will share your questions and comments with those employees that can help you with your questions. Keep in mind that e-mails may be read by other Internet users. For this reason, we ask that you call us to talk about things that you feel are private.
If Our Privacy Policy Changes:
We may change our privacy policy from time to time. We will send our members a Notice about our privacy policy at least once a year. We will also put an up to date privacy policy on our web site at: www.vhptn.com.
No person, on the grounds of race, color, national origin, sex, age, religion, or disability, shall be treated differently or denied the benefits of any program or service provided by VHP Community Care.
